Upon arrival, Jessica Marin will welcome you to your appointment. She will begin by applying a numbing cream, taking this time to also schedule your second session. During this period, Jessica will provide detailed explanations about aftercare and walk you through what to anticipate in the upcoming weeks. This process typically lasts about 20-30 minutes.

Please note that if you arrive late, we may not have sufficient time for numbing or rescheduling and your deposit will be forfeited.

Once the numbing process is complete, Jessica Marin will guide you to the next stage of your appointment.

For optimal results, we recommend avoiding caffeine and workouts on the day of your appointment. You can resume your normal caffeine intake after the completion of your appointment.

Please keep in mind that permanent makeup is a multi-session process, with the final touches being applied during the second session. Your journey is not complete until after your second session. Please note that deposits are non-refundable.

I realize that there is a potential for discomfort during the procedure and healing process, as well as the possibility of bleeding and swelling and/or an allergic reaction to the products used. I understand that no warranty or guarantee has been made to me as a result of the semi permanent makeup procedure. I understand that the tattooing is considered semi-permanent and will fade with time. I understand that a tattoo can only be removed with a laser or saline. I understand that the healing process varies for everyone. Pigment loss or color change normally happens in most cases during the first week. I will return for my touch up session no earlier than 6 weeks, and no later than 10 weeks after my initial session (when skin is completely healed).

I understand that eclipse cosmetic  tattoo has the right to stop contact with any client at any time, for any reason. Eclipse cosmetic tattoo has the right to stop the service at any time, for any reason. I believe that I have been given sufficient information to give this informed consent.

As a client, you have a responsibility to inform the technician of all possible concerns. I understand that I must inform the technician of all medications being taken by me, even though I have it written on my Medical History Form. I understand that it is my responsibility to advise my technician of any concerns I may have before they begin the procedure. I am free from drug and alcohol use or any other substances. I am NOT pregnant or breastfeeding. I have no known allergies to dye, nickel, or any topical anesthetics. I understand that taking before and after pictures is a condition of such procedures. I hereby forever release and discharge Eclipse Cosmetic tattoo/ Jmarin cosmic tattoo LLC from any and all claims, actions, and demands arising out of use of said photographs for the website and social media.

I have read and understand the Aftercare Treatment instructions and recommendations, and I understand my responsibility to follow them to ensure proper healing of treated area.

I acknowledge that any information contributed by me is true, to the best of my knowledge, and that the present condition of the area that has been treated or will be treated is stated on this record. I fully understand that Jmarin Cosmetic  LLC only provides beauty services; there is no medical treatment involved.

CLIENT further agrees to indemnify and hold harmless Jmarin Cosmetic tattoo LLC from any claim of liability, losses, damages, or any expenses whatsoever as a result of any claims, demands, damages, costs, or judgements including, but not limited to, claims based on negligence against it that may arise in connection with the services performed by an independently contracted technician. This agreement is intended to be an addendum to any previous conditions, releases, or hold harmless agreements, in written form, verbal, or manually communicated between Studio Aurora and its client in connection with the permanent makeup procedures. The client has been given the opportunity to attain reasonable understanding of this agreement, including the opportunity to ask questions, either by written, verbal, or manual communication prior to the signing of this document.

I release Eclipse Cosmetic Tattoo and its representatives and subsidiaries of all claims from injury, seen or unseen that may result as a result of this procedure. I fully understand the questions, terms, and conditions of the Disclosure and Release Agreement. I accept to and waive all my rights for any claim against the technician at Eclipse Cosmetic Tattoo for any reason whatsoever. I certify that this Disclosure and Release Agreement was completed by me and that all entries in it and information are true and complete to the best of my knowledge.

I realize that with any beauty service, there may be risks which must be understood. I will be fully responsible for any and all results which may arise from these beauty services. I do hereby agree to hold, Eclipse Cosmetic Tattoo, Jessica Marín free from any and all claims or suits for damage, for injuries or complications resulting from any beauty service provided by Jmarin Cosmetic tattoo LLc

The nature and purpose of the beauty services, the risks involved, and the possibility of complications have been fully explained to me. I understand that no guarantee or assurance has been given by anyone as to the results that may be obtained.
